Great opportunity exists for a Therapeutic Mentor to join our dedicated team of professionals! Therapeutic Mentoring is a structured and goal-oriented strength-based support service provided to youth under age 21 at home or in a community setting. Therapeutic mentoring services include supporting, coaching and training the youth in age-appropriate behaviors, interpersonal communication, problem solving and conflict resolution skills. Youth are referred from outpatient therapists, in home therapists and intensive care coordinators who provide treatment plans to guide the goals of therapeutic mentoring.

Responsibilities:

  • Travel to youth's home and throughout the community
  • Engage youth in home, school and/or community activities to teach new strategies and skills that are linked to the youth's treatment plan
  • Support the youth in recognizing individual skills and strengths
  • Help identify a natural support system and foster independence
  • Regular coordination with collaterals providers, including schools, state agencies, outpatient therapists, in home therapists, intensive care coordinators and other team members
  • Maintain accurate clinical charts and follow all clinical documentation timelines and requirements
  • Participate in weekly supervision and staff meetings
  • Meet agency productivity requirements

Schedule Monday through Friday 11am-7pm

Required Skills
  • Valid driver's license and a safe driving record
  • Access to a personal vehicle for client transportation
  • Bilingual is a plus; Spanish
  • Computer proficiency required

Required Experience
  • Bachelor's or Associate's Degree with a concentration in Psychology, Human Services or related fields
  • Must be 21 years of age or older
  • Must have at least 1 year of previous experience working with children and adolescents with mental health issues with their families
  • Experience utilizing Electronic Health Record preferred
