Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

The Stroke Performance Improvement Coordinator will review every Stroke One activation and every patient discharged with a primary diagnosis of stroke for opportunities for improvement. Data will be abstracted and entered into the databases including the stroke core measure and opportunities will be shared with respective departmental leadership. Action plans for improvements will be developed and reviewed with appropriate leadership. Data will be collected and collated and presented to various Committees.

Essential Functions and Responsibilities

  • Proactively contributes to Eskenazi Health's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County; models Eskenazi Health values
  • Assists direct patient care of the stroke patient in the Emergency Department upon all Stroke ONE activations while in-house; assists with documentation of Stroke Narrator to assure physician response times and patient assessment are recorded appropriately
  • Is a technical expert and resource with the stroke patient population or is able to provide access to expert care as needed
  • Functions as a clinical expert, clinical consultant and resource to nursing unit staff to facilitate consistent quality patient care
  • Serves as a resource and instructor for neuroscience education offerings
  • Supports regulatory readiness for units via rounding, coaching and monitoring; is aware of standards and regulations applicable to clinical units
  • Assists Clinical Educator with special programs within area of expertise
  • Identifies and coordinates performance improvement activities with direction; initiates processes to resolve clinical problems, PI project with direction including documentation compliance, death reviews, complication reviews, transfers and data validation
  • Maintains current working knowledge of external and internal quality indicators and standards as it pertains to the neuroscience patient
  • Monitors the adherence to protocols and variances
  • Assists the Stroke Program Director in the development, implementation and evaluation of a quality plan, which is multidisciplinary and patient outcome focused
  • Creates spreadsheets, databases and power points as needed

Job Requirements

  • Active RN license in the State of Indiana, or the ability to obtain active licensure through application
  • Bachelor's degree in nursing
  • CPR, ACLS, ENLS certifications
  • Current specialty Board Certification, such as SCRN, CNRN, CCRN or CEN or relevant and approved certification thru ANCC preferred or must earn within 2 years of accepting position
  • 5 years of clinical nursing experience in Emergency Department of Critical Care preferred
  • Knowledge of stroke education requirement as it relates to hospital programs with the Joint Commission, State Board of Health and OSHA
  • Ability to consult and teach departments regarding age-appropriate and cultural needs of neuroscience patients
  • Competency assessment, educational programming, delivery methodology expertise
  • Ability to function independently; possesses strong decision-making and problem-solving skills
  • Exhibits interpersonal relations, conflict resolution, education and organizational planning skills
  • Ability to work independently
  • History of skill development and learning to maintain expertise
  • Maintains expertise in those areas programmatically responsible for core stroke clinical practices, protocols, patient safety, clinical policies, procedures and programs

Knowledge, Skills & Abilities
