Description: The Judge Group is currently seeking a Social Worker in Orange, CA!

Social Work Case Manager

As a Social Work Case Manager, you will assess prospective members for various programs. Your responsibilities will include developing, implementing, monitoring, modifying, and documenting care plans. Additionally, you will accept referrals for case management and make recommendations for the appropriate level of care and necessary measures to successfully manage care.

Key Responsibilities:

• Collaborate with an interdisciplinary team to identify and address psychosocial needs, including financial issues, transportation, and community support services.
• Coordinate the psychosocial needs of members, utilizing community resources and support when appropriate.
• Work with external utilization management personnel and the community to identify members who could benefit from case management services.
• Provide social work consultation and education to members and their families, acting as an advocate as needed.
• Analyze assessments to identify individual needs and appropriate resources.
• Monitor services provided to members, ensuring necessary and available resources are utilized.
• Anticipate members' needs, provide support, and maintain a role as their liaison while promoting independence and decision-making.
• Make referrals based on assessed needs.
• Collaborate with members, family members, significant others, physicians, and other healthcare providers to achieve goals identified in the care plan.
• Proactively identify members appropriate for care management.
• Initiate appropriate follow-up care and develop individual care management service/care plans, incorporating assessment, education, and resources.

Minimum Qualifications:

• Master's degree in social work required.
• 2 years of experience working with the gerontology population required.
• Experience with behavioral health, health facility, health plans, counseling, case management, home health or hospice required.
• An equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.
• This is a patient facing role, so vaccinations and clinicals are required.

Preferred Qualifications:

Bilingual in English and one of the following languages (Arabic, Farsi, Chinese, Korean, Spanish, Vietnamese).
