How you can make a bigger difference:

PURPOSE:

The Jewish Board's Adult Residential Division supports adults with serious and persistent mental illness to live in the community with as much independence as possible. Using a non-judgmental, harm-reduction approach, staff model and guide residents to create and meet personalized recovery goals, develop independent living skills, and live in a healthy and secure manner. Our work is guided by principles that underscore the critical importance of stable housing, use a culturally competent and anti-racist lens, and respect the dignity and legitimacy of each individual's journey.

POSITION OVERVIEW:

The Social Worker is responsible for supplying necessary clinical intervention within the realm of the program. Provide cultural competent and sensitive services. As part of therapeutic team within the Agency, work closely with the divisions' medical, psychiatric and psychology staff, to ensure the appropriate treatment interventions for the client.

KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):

  • Coordinate all services for consumers, including day programs, medical care, transportation, summer camps, summer programs, recreational, and community inclusion (Person Centered Planning).
  • Liaise between Department and consumers family members, and relevant community members. Provide family limited counseling.
  • As Team Leader, lead quarterly case conference, and prepare minutes. Prepare integrative treatment plan, based on all assessments from the other professionals and Direct Care Workers.
  • Lead weekly team meetings. Keep minutes of team meetings and ensure follow-up as necessary from meetings.
  • Conduct quarterly school visits to insure proper placement, progress in program and coordination of goals.
  • Attend annual case conference at day program or school.
  • Train Direct Care Workers as necessary.
  • Participate in audit preparation.
  • Work with intake coordinator on intake process for new consumers.
  • Work with Direct Care Workers to enhance communication and cooperation between them and consumer families.
  • Complete paperwork in a timely manner.
  • Other duties as assigned.
EDUCATIONAL / TRAINING REQUIRED (List all that is required to achieve this position):
  • Master's degree in Social Work (MSW) and LMSW/LCSW with a minimum of one year of relevant experience with similar population.

COMPUTER SKILLS REQUIRED (List the computer skills needed):
  • Familiarity with Microsoft Office suite of products (Excel, Word, PowerPoint, etc.).
  • Ability and willingness to learn new systems and application software.

WORK ENVIRONMENT / PHYSICAL EFFORT
  • Ability to travel to visit residents in apartments or to escort residents in the community.
  • Ability to lift 25 pounds to assist residents and to manage stairs in walk-up buildings as needed.
