Location: Orange, CA
Salary: $23.00 USD Hourly - $30.00 USD Hourly
Description: Judge Healthcare is seeking a Personal Care Coordinator in Orange, CA!

Key Responsibilities:

• Process routine WCM referrals, AMRs, and ICTs within appropriate timeframes for all members.
• Collaborate with health networks to ensure timely receipt of members' clinical information.
• Coordinate with OCHCA CCS to address health networks' need for necessary clinical information.
• Work with Case Management staff, OCHCA CCS, and other healthcare team members to facilitate medical care and expedite resolution of member concerns.
• Perform outreach to members to coordinate the completion of initial and annual HNAs or HRAs.
• Review member concerns or questions that may require additional support or outreach.
• Facilitate communication of initial care plans to primary care providers (PCPs), health networks, and members as needed.
• Facilitate referrals to behavioral health services.
• Assist members in understanding and accessing benefits under Medi-Cal and CCS.

Minimum Qualifications:

• High School diploma or equivalent.
• 1 year of experience working with pediatric members in a member service capacity.

Preferred Qualifications:

• Bachelor's degree in healthcare management or related field.
• 1 year of experience in Health Maintenance Organization (HMO), Medi-Cal, and/or health services.
• Bilingual proficiency in English and Arabic, Chinese, Farsi, Korean, Spanish, or Vietnamese.
