Position Summary:
The MDS Coordinator is responsible for the day-to-day coordination and oversight of all aspects of the RAI and Care Planning process in accordance with Federal and State
regulations.
Daily duties:
- Scheduling of MDS assessments
- Completion of MDSs
- Completion of RAPs
- Complete/update Care plans
- Utilize the Daily Care Plan Update sheet and work collaboratively with the nursing team to ensure implementation.
- Attend daily meetings as assigned
- Conduct Resident Care Conferences per facility schedule
- Communicate with interdisciplinary team members
- Track admissions and discharges
- Attend morning meeting/clinical report as assigned
- Computer Application/Data Entry
Weekly duties:
- Transmit MDS's to State
- Attend weekly resident "At Risk" meetings
- Work collaboratively with nursing team to properly utilize and implement the MDS Alert Sheet.
Monthly duties:
- Complete monthly calendar for MDS completion and Resident Care Conference 2. Attend and participate in the Quality Assessment and Assurance meeting
- Attend and participate in the QI/QM investigation meeting.
- Attend all monthly staff meetings
Quarterly Duties:
- Attend facility communication meetings
- Attend Regional MDS Coordinator meetings
- Other duties as directed by the Director of Nursing/Administrator
Recommended Minimum Position Qualifications:
Education:
- Graduate of an approved LPN/RN program.
- RN preferred.
Professional Experience:
- MDS Coordinator experience preferred.
Certification/Licensure:
- Actively licensed as an LPN/RN in assigned state(s) of employment.
