Position Purpose Summarizes the primary purpose of the position.
Responsible for monitoring and improving the level of care provided and helping build census. The position is also
responsible for patient care, clinical outcomes, and the implementation and oversight of the quality assessment and
performance improvement program to include medical oversight for the wound care program.
Essential Functions Lists up to five duties, indicating the largest percentage of time first, and the approximate
percentage of time spent on each over the course of a year, totaling 100%. The percent of the time is an estimate and
may fluctuate based on time of year, department, etc.
1. Patient Care: Manage patient admissions, and daily rounding, and ensure appropriate consults and transfers.
Discharge patients when ready, ensuring a smooth and efficient process. Provide high-quality care to patients,
addressing their medical needs and concerns. Be responsible, on a case-by-case basis as the need arises, for
taking all actions necessary to facilitate the appropriate transfer of a patient to or from another medical
facility to gain access to skilled medical care.

2. Quality Improvement: Assist with quality improvement projects aimed at enhancing patient care and
operational efficiency. Identify areas for improvement and implement strategies to achieve better outcomes.

3. Meetings and Collaboration: Attend necessary meetings with medical staff, the Medical Executive
Committee, and the Utilization Review. Collaborate with colleagues to ensure coordinated and
comprehensive patient care.

4. Continuing Medical Education: Participate in continuing medical education to stay current with the latest
medical advancements and best practices. Engage in professional development opportunities to enhance
clinical skills and knowledge.

5. Licensing and Credentialing: Maintain all necessary credentials with up-to-date licensing and credentialing.
Ensure compliance with all regulatory and accreditation requirements.
