Location: REMOTE
Description:

We are seeking a highly motivated and independent Registered Nurse (RN) to join our team as a Field Assessor. This role involves conducting in-person assessments for PCA (Personal Care Assistant) and MDC (Major Diagnostic Category) services. The position requires travel within Union and Hudson Counties, with mileage reimbursement provided.

• Job Type: Temp to perm/ contract to hire
• Location: North New Jersey (Union and Hudson Counties)
• Schedule: Monday to Friday, 8:00 AM-5:00 PM or 9:00 AM-5:00 PM
• The training is for the first two weeks

Responsibilities:

• Perform patient assessments to determine appropriate services using established guidelines.
• Evaluate the necessity, appropriateness, and efficiency of services provided.
• Develop and coordinate care plans tailored to patient needs.
• Collaborate with patients, families, physicians, and other healthcare professionals to ensure optimal care delivery.
• Monitor patient care activities and outcomes for effectiveness and appropriateness.
• Advocate for members and facilitate coordination across care settings.
• Educate members about additional resources, such as SNP and MLTSS, when appropriate.
• Accurately document all assessments and interactions following organizational policies.
• Leverage technology and communication tools to interact with multidisciplinary teams.
• Contribute to quality improvement initiatives to enhance member outcomes.

Qualifications:

• Active RN license in New Jersey.
• Valid driver's license and reliable transportation.
• Experience in Home Health or Medical-Surgical settings.
• Proficient in technology and comfortable with documentation systems.
• Self-motivated, organized, and detail-oriented.
• Strong ability to work independently.

Additional Information:

• This is a remote field role; there is no office attendance requirement, but travel for member assessments is mandatory.
• Average of 3-4 member visits per day.
• Mileage reimbursement is provided.
• This role is open to RNs only.
