Provide medication management through an evaluation of medications for patients to ensure safe, effective utilization that
is tailored to specific medical conditions. Work with patients, providers, nurses, and other health care professionals to
provide access to specialty medicationsand ensure optimal patient outcomes. We approach patient care as a team and
support each other as needed or directed to ensure the necessary patient care responsibilities are completed each day.
This role is an exempt position with the potential for a hybrid on-site/work from home work model.

Responsibilities:

Pharmacy Practice Involvement:

  • • Proactively maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in services, seminars, and conferences.

  • Actively participate in clinical meetings

  • Participate in training staff as directed

  • Cross train for other roles to provide periodic support as directed based on business needs

  • Participates in Clinical Subcommittees at MGBSP

  • Precepts students/residents as directed

Clinical Practice Expectations:
  • Ensure all drugs are appropriately indicated, effective, safe and convenient for the patient
    Identify, resolve and prevent drug therapy problems
  • Document safety events at time of discovery through MGBSP safety event tool and assist in resolution under guidance of the Safety Officer
  • Provide immediate and scheduled patient counseling and respond to inquiries from patients, family members, and health care professionals concerning drug therapy in a timely manner
  • Document therapeutic interventions based on patient and medication therapy evaluation
  • Facilitate medication access by assisting with prior authorizations, drug therapy appeals, financial assistance, and other Patient Care Coordinator activities as directed.
  • Provide On-call support on a scheduled rotation

Perform patient evaluation as it pertains to medication management:
  • Obtain patient histories (medication, allergy, disease) and apply these along with other patient data in the pharmaceutical care evaluation
  • Evaluate and document patient outcomes at each encounter
  • Provide patient education on medications and disease states in individual or group settings
  • Collect analyze, interpret and evaluate appropriate data relative to medication therapy
  • Ensure that the patient understands the specific goals of therapy and that the patient and providers are working to achieve these goals
  • Identify, resolve, and document patient specific drug therapy problems and issues in the patient's best interest
  • Evaluate the significance of signs and symptoms relative to disease processes and make recommendations for the modification of drug therapy
  • Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy

Qualifications
Qualifications

• Degree from an accredited college of pharmacy


Level of education preferred for this job: Pharm D


Experience (2 years) managing patients' specialty medication needs or, Clinical Pharmacist hospital practice experience (2 years) preferably in one or more specialties


Excellent verbal and written skills


Proficiency with Microsoft Office

Skills/Abilities/Other Details:

• License/Certification/Registration: Current State pharmacy license


Preferred License/Certification/Registration: CSP, BCOP, BCPS or similar


Minimum Experience: Three years' experience as a clinical pharmacist with direct patient care


Preferred Experience: Specialty Pharmacy and/or, Pharmacy Residency Program (PGY1 or 2)


Additional Requirements: Excellent customer service and communication skills, proficient in the English language, ability to manage time and people effectively, good attention to detail, ability to follow policies and procedures.

• Commitment to applying individual clinical skills in collaborative team environment to achieve patient-specific and program-specific goals.
