Description: We are seeking a Case Manager RN for a Medical Clinic in Marysville, WA!
Job Description:
Job Title: Case Manager RN
Location: Marysville, WA 98271
Start & end times of shift: 7:30-4 pm
Days of the week they'll work: M-F
Type of certification required: There are no certifications required for the role (but CCM or Palliative Care Certification would be a major plus!)
Rate: Depends on experience
Work type: Contract (possible temp to hire)
This job will have the following responsibilities:
- Hospital Transition calls for Medicare Advantage patients
- Care Coordination for complex, high-risk patients
- Motivational interviewing with patients and families
- Palliative Care conversations when appropriate
- Participation in Interdisciplinary Team meetings
- Ambulatory Care
- Outpatient Care Coordination
- Difficult conversations
- Care Management
- Hospital Transition work
