Description

Are you passionate about helping people? Do you have a talent for listening, Problem solving, and advocating for others?

If so, we have an exciting opportunity for you to join our team as a Care Navigator.

A dedicated team that values empathy, innovation, and collaboration. Your contributions will not only enhance the lives of those you serve but also inspire hope and resilience within our community. Together, we'll navigate the path to well-being, one person at a time.

The Care Navigator will provide in-person outreach services to individuals in the community who are involved with the Criminal Justice system and have histories of behavioral health and/or substance use disorders, helping them to connect to needed resources and to overcome barriers impacting their care.

Who are we?

Behavioral Health Justice Involvement Program

This program seeks to establish a sustainable model for engaging adults involved in the criminal justice system with mental health and substance use disorders to improve health outcomes, reduce recidivism, decrease fatal overdoses and support effective transitions to needed services in the community.

Behavioral Health Network (BHN) is a growing non-profit community behavioral health agency that has been providing services to children, adults, families and communities in Western Massachusetts since 1938. BHN provides comprehensive, outcome-driven behavioral health care. We are dedicated to offering high quality, affordable and culturally appropriate care to people of all ages and income levels in our constituent communities.

What you'll do:

As a Care Navigator, you will provide in-person, telephonic, and Telehealth support to individuals, fostering a collaborative approach within our integrated health model. Your role will involve meeting clients in 24-hour facilities-such as jails, hospitals, and substance use treatment centers-and maintaining regular contact to facilitate discharge planning and re-engagement with outpatient services.

You'll serve as the primary point of contact for your caseload, ensuring that each individual's needs are met. Collaborating with providers and under the guidance of the Clinical Supervisor, you will complete comprehensive assessments and individualized support plans, including safety and disenrollment plans.

Engaging with all relevant parties-such as jail personnel, probation officers, healthcare providers, and natural supports-you will ensure a cohesive approach to care, always with the individual's written consent. Additionally, you will leverage your knowledge of community services to assist clients in accessing resources like health insurance, housing, financial assistance, and educational support.

In times of crisis, you will provide management support alongside the Clinical Supervisor and the Emergency Services Program, participating in a 24/7 on-call response team. Your work will empower individuals to achieve improved health, independence, and community integration.
