BC Forward is looking for Care Coordinator II (Spanish/Creole) - Remote in Tampa, FL 33634

Position Title: Care Coordinator II (Spanish/Creole) - Remote

Locations: 8745 Henderson RD, Tampa, FL 33634
Anticipated Start Date: 01/06
Expected Duration: 6 months
Job Type: Contract with possible extension
Shift: Monday - Friday 9:00 a.m. - 6:00 p.m. EST
Pay Rate: $25.00/hr on W2.
Note: Candidates residing in the Tampa Bay area are preferred, as there may be occasional in-person meetings (quarterly). While attendance is not mandatory, the ability to participate is highly desirable.

Job Description:
Position Purpose:
The role supports care management activities by ensuring healthcare services are effectively delivered to members through collaboration with providers and community partners. This position promotes continuity of care and member satisfaction by engaging with members via phone or home visits to address their care plans, resources, and overall needs.
Day-to-Day Responsibilities:

  • Conduct outbound quarterly follow-up calls to assigned members, completing a series of follow-up questions as part of their care management.
  • Manage a designated caseload of member cases each month.
  • Perform up to 3 attempts to contact each member (Day 1, Day 2, Day 3). If unsuccessful, collaborate with the member's Primary Care Provider (PCP) to verify or obtain updated contact information.
  • Accurately document all outreach attempts, outcomes, and member interactions in the appropriate systems in compliance with standards.
  • Participate in stretch assignments or additional tasks based on team volume and business needs.
Performance Expectations and Metrics:
  • Maintain a daily target of 50-75 outbound calls to members.
  • Demonstrate consistency in following the outreach process, including documentation and follow-up.
  • Ensure timely and accurate updates of member records in compliance with regulatory and organizational standards.
  • Exhibit adaptability to stretch assignments and meet performance goals set by leadership.
Required Skills/Experience:

  • 2+ years of experience in healthcare or managed care.
  • Must be comfortable with outbound and inbound calling.
  • Ability to multitask, including completing calls while taking detailed notes simultaneously.
  • Adaptability to change and flexibility in a dynamic work environment.
  • General technology proficiency, including Excel, Outlook, Word, and Teams.

  • Preferred Skills/Experience:

  • Bilingual (Spanish or Creole).
  • Pediatric care experience.
  • Previous work-from-home experience.

  • Education Requirement:
    • High School Diploma (HSD) or equivalent experience.
    Key Responsibilities:
    • Conduct outreach to members through phone calls or home visits to discuss care/service plans, next steps, resources, and any concerns, providing ongoing education and support as needed.
    • Coordinate care activities in alignment with the member's care/service plan, collaborating with healthcare providers, community organizations, and caregivers to adjust plans based on progress or changes.
    • Serve as a point of contact for member and provider inquiries, requests, and concerns related to care/service plans.
    • Communicate with care managers, practitioners, and other relevant stakeholders to facilitate services and ensure continuity of care.
    • Assist with assessments/screenings to document members' care needs as required.
    • Maintain accurate and up-to-date member records in compliance with state, regulatory, and contractual standards, distributing documentation to providers when necessary.
    • Adhere to policies, standards of practice, and regulatory guidelines.
    • Identify member needs and make appropriate referrals to Care Managers, Disease Managers, and community-based organizations.
    • Provide education to members about benefits and available resources.
    • Perform additional duties as assigned.
    Qualifications:
    • High School Diploma or GED required.
    • 1-2 years of relevant experience in a related field.
