PURPOSE OF THIS POSITION

The primary purpose of your job position is to assist with the planning and executing of social, recreational and educational programs for the residents in accordance with Independence House policy and procedure, Independence House ethics statement and resident rights, subject to the employee handbook, and as directed by the Activity Coordinator.

JOB DUTIES/RESPONSIBILITIES

Duty 1: Ability to think creatively, plan activities, manage multiple projects, direct volunteers, and use time efficiently. Ability to responsibly and effectively use supplies, resources, and activity-related equipment. Work within budgeted hours. Keep activity areas neat and orderly.

Duty 2: Ability to accurately assess resident's individual interests and needs, translating them into the inclusion of every resident within your caseload. Identify need for new or different resident activities/services. Include and encourage family involvement. Refer to Social Services or Chaplain when indicated. Report condition or behavior changes to the charge nurse.

Duty 3: Ability to communicate effectively with residents, neighborhood associates, family members, and volunteers. Work within the Neighborhood team parameters.

Duty 4: Ability to accurately complete and maintain required documentation which may include: activity assessments, participation records, the MDS, Care Plans, progress notes, and discharge summaries. Documentation will be done in a timely manner and A.A. will have working knowledge of the computerized record-keeping systems.

Duty 5: Displays service excellence and the standards or behavior as evidenced by practicing the mission, vision, and values of the organization to promote resident satisfaction.

Duty 6: Complies with organizational policies, procedures, and practices to ensure quality job performance and regulatory compliance.

REQUIRED QUALIFICATIONS

  • Able to communicate with residents and encourage them to participate in activity programs.
  • Ability to apply commonsenseunderstanding to carry out detailed but uninvolved written or oral instruction.
  • Ability to deal with problems involving a few concrete variables in standardized situations.
  • Emotional stability to accept residents' problems and a desire to be of service to them. Able to undertake a variety of changing projects as required by many different residents. Possess imaginative and creative ability plus sincere desire to work with the aged and infirm.
  • Positive service-oriented interpersonal and communication skills required.
  • Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status. Must be able to interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs and to provide the care needed as described in the area's policies and procedures
PREFERED QUALIFICATIONS
  • High school diploma or equivalent preferred
  • One year experience working in a long-term care facility or related field.

This position is classified "at risk" for possible occupational exposure to blood borne pathogens (HBV, HIV, etc.)
